Where appropriate (e.g., disk controllers), list the model, type and quantity of all devices (e.g. 4 3990-2).

The form below is pretty comprehensive, but there may be special requirements you have or questions you want to ask that don't fit the basic form.  With that in mind, you will find a place at the end of the questionnaire to add any extra information or raise any special questions that you think may bear on your forward migration.
